Don't eat anything. Only when food intake completely ceases does your body activate its second program, one it otherwise never starts. This page explains the four levels of fasting, showing day by day what happens within you, and distinguishes between the paths for women and men.

The four depths of fasting

Intermittent fasting The exercise is: twelve to sixteen hours of rest daily, suitable for everyday life, doable without instructions. Alkaline and intermittent fasting The gentle approach is: five days of severely reduced intake, around 800 kilocalories, followed by about 500 kilocalories, purely plant-based and low in protein. Therapeutic fasting The core principle is: five to twenty days without solid food, a maximum of 250 to 500 kilocalories from juice, broth and some honey, plus at least 2.5 liters of water and tea. Water fasting This is the deepest form and belongs exclusively in a facility with blood tests.

The difference between these stages is not the quantity, but the duration. As long as even small amounts of food are received, your body maintains its daily routine. It waits for more supplies. Only when nothing is received for several days does it abandon this waiting and switch completely to the new mode.

What happens day after day

The day of relief Before that, about 1,000 kilocalories of light food are consumed, with no coffee, no sugar, and no alcohol. This one day halves the impact of the second fasting day. Day 1 It begins with bowel movements, followed by only water and tea. The sugar stores are depleted, and with every gram of sugar, three grams of water are lost: the first two kilograms are not fat.

Day 2 is the fasting crisis. Headache, chills, irritability. It feels like detoxification, but is largely due to mineral loss because the kidneys excrete more sodium without insulin. A cup of strong vegetable broth, a pinch of salt in the water, and a walk often resolve this within an hour.

Day 3 is the turning point. Ketosis is established, your brain is largely running on ketones instead of sugar, and at that precise moment, hunger disappears. In the largest study on this topic, 93 out of 100 participants reported experiencing no hunger. From day 4 The calm depths follow: Blood pressure drops noticeably, inflammation levels decrease, and the mind becomes clear. From day 8 Fasting belongs in experienced hands.

What is being pushed aside

Fasting is not a weight-loss tool, even though weight is lost. It's a recalibration of systems that have been pushed in a certain direction for years: blood pressure, blood sugar and insulin, liver and fat metabolism, low-grade inflammation, the gut, and last but not least, appetite itself. After fasting, a carrot tastes different. That's not just your imagination; that's the real benefit.

The largest study on this topic followed 1,422 people through five to twenty days of fasting, nearly sixty percent of whom were women. The average systolic blood pressure dropped from 131.6 to 120.7 mmHg, and waist circumference decreased by 4.6 to 8.8 centimeters. Less than one percent reported an adverse effect. Of those who presented with symptoms, 84 percent felt better afterward. For context: this was an observational study conducted in a fasting clinic without a control group.

Autophagy, honestly speaking

Cellular cleansing is well-documented; it was even awarded the Nobel Prize in Medicine in 2016. The honest part: the figures circulating everywhere are not based on human data. Directly measuring autophagy in a living person is technically very difficult; almost all figures come from animal studies or are derived from indirect indicators. The state associated with increased autophagy is reached after about sixteen to twenty-four hours and deepens the longer the fast lasts. It's a process, not a fixed threshold.

Women's fasting

What usually throws the female cycle off balance isn't a single week of fasting, but rather the small, daily calorie deficit over several weeks. An axis runs between the brain and the ovaries that regulates the cycle; a messenger substance called kisspeptin triggers this process, and women produce significantly more of it than men. Therefore, the timing of the fasting week is crucial: begin between cycle days 6 and 13, not during menstruation and not in the second half of the cycle.

It's common for the next period to be delayed or even skipped once, and this usually happens only once. Women feel the cold much more intensely during fasting; this is the most frequent reason for abandoning it: hot water bottle on the liver, warm broth instead of cold water, wool socks. After menopause, timing becomes less of a concern, and sleep and bone health take center stage.

Men's fasting

The male body lacks the feedback loop that stops the menstrual cycle in women as soon as energy becomes scarce. It signals deficiencies later and more subtly. This is precisely why men usually tolerate multi-day fasts well and often extend them further. The three pitfalls are going it alone, ambition, and prematurely ending the fast.

One point is almost always overlooked: Uric acid levels rise during fasting because ketones and uric acid share the same excretory pathway in the kidneys. Anyone who has already had a gout attack can trigger another one after a week of fasting. The fat surrounding the internal organs, on the other hand, reacts strongly: Measure your waist circumference, not your weight. After fasting, a diet of around 1.6 grams of protein per kilogram of body weight and two strength training sessions per week are recommended.

The setup days are crucial

The most dangerous part isn't the fasting itself, but the end. After several days without food, potassium, magnesium, and especially phosphate are depleted, even if blood levels still appear normal. If a large amount of food is then suddenly consumed, insulin levels spike and draw these minerals into the cells. This creates a sudden deficiency in the blood, which can lead to symptoms ranging from weakness and confusion to cardiac arrhythmias. For every day of fasting, there should be approximately one-third of a refeeding day, but at least three. The first bite should be a stewed apple.

Those who should not fast

Multi-day fasting is contraindicated during pregnancy and breastfeeding, in cases of underweight and emaciation, a history of eating disorders, uncontrolled hyperthyroidism, advanced liver or kidney failure, dementia, and for children and adolescents. Those taking medication should not fast alone: blood pressure medication, blood sugar medication, insulin, blood thinners, and lithium must be adjusted. Fasting should also be medically supervised in cases of type 1 diabetes, gout, gallstones, cardiac arrhythmias, and any cancer treatment.

This page does not replace medical advice. Fasting for several days is an intervention in the metabolism.

Fasten Nicht weniger essen. Nichts essen.
